Registration

EBI conducts camps, clinics, and lessons at various locations around the Eastside. For this reason it is necessary to make sure you register using the correct form and procedure.

EBI Program registration:

For EBI Private/Semi Lessons or Classes and Camps there are three ways you may register:
1. Print forms and bring with payment to first lesson or class.
2. Print the forms, fill out and send via mail with payment.
3. Print, Fill out and email your form with payment information.
Registration form and Waiver form is required for all classes, camps and lessons. Send registration and waiver forms to:
